Mentees

• Completed Application
• Personal interview
• Three (3) recommendations: One (1) Teacher and Two (2) Non-family
• Parent / guardian written participation approval; including approval to consult with teachers, guidance counselors, coaches, etc.
• Liability Waiver and Medical Release
• Complete LIFT 4:10 Orientation Training
